A

seronegative inflammatory arthropathy affecting preferentially the axial skeletal + large proximal joints

Describe the aetiology of Ankylosing Spondylitis

A

UNKNOWN
> 90% of cases are HLA-B27 positive
Infective triggers + antigen cross-reactivity with self-peptides have been hypothesised

Describe the pathophysiology of Ankylosing Spondylitis

A

Inflammation starts at the entheses
Persistent inflammation leads to reactive new bone formation
Changes begin in the lumbar vertebrae + ascend

List 6 symptoms of Ankylosing Spondylitis

A

Lower back + sacroiliac pain
Disturbed sleep
Sx of asymmetrical peripheral arthritis
Pleuritic chest pain (due to costovertebral joint involvement)
Heel pain (due to plantar fasciitis)
Non-specific sx (e.g. malaise, fatigue)

Reduced range of spinal movement (esp. hip rotation)
Reduced lateral spinal flexion
Tenderness over the sacroiliac joints

A

Schobers test
2 fingers placed (parallel at different spinal levels) on pts back ~10 cm apart
Pt is asked to bend over
Distance between the 2 fingers should increase by > 5 cm on forward flexion
