How do we diagnose anahylaxis?
look for:
acute onset
lifethreatening airway and/or breathing and/or circulatory problems
usually skin changes
What is the first step in management of anaphylaxis?
call for help
lie the patient flat
raise their legs.
Then give adrenaline.
What is the management of anaphylaxis when help arrives?
airway and high O2
IV fluid challenge
Chlorphenamine
Hydrocrotisone.
Attach monitoring.
How should adrenaline be given in anaphylaxis?
IM unless experienced enough to give IV.
How much adrenaline do we give for adult anaphylaxis?
500 micrograms IM (0.5ml - 1:1000)
repeat after 5 mins if no response.
How much adrenaline do we give for child over 12 anaphylaxis?
500 micrograms IM (0.5ml - 1:1000)
repeat after 5 mins if no response.
How much adrenaline do we give for age 6-12 anaphylaxis?
300 micrograms IM
(0.3ml - 1:1000)
repeat after 5 mins if no response.
How much adrenaline do we give for under 6 anaphylaxis?
150 micrograms IM
(0.15ml - 1:1000)
repeat after 5 mins if no response.
What fluid challenge do we do in adult anaphylaxis?
500-1000ml
What fluid challenge do we do in child anaphylaxis?
crystalloid 20ml/kg
How do we give chlorphemine in anaphylaxis?
IM or slow IV
How do we give hydrocortisone in anaphylaxis?
Im or slow IV.
How much chlorphemine do we give in adult anaphylaxis?
10mg
How much chlorphemine do we give in child 6-12 anaphylaxis?
5mg
How much chlorphemine do we give in child 6m-6yanaphylaxis?
2.5mg
