Morphine Sulfate (Roxand, MS Contin)

A

Dosage: A: STEMI: Initial dose: 2-4mg slow IV (1-5min). Repeat dose: 2-8mg at 5-15min intervals. NSTEMI/Unstable angina: 1-5mg IV push if symptoms not relieved by NGT with caution. PEDI: 0.1-0.2 mg/kg one dose IV/IO/IM/SC.Max dose 5mg.

A

Dosage: A: 0.4-2mg IV/IO/IM/SQ/intranasal (Diluted); minimum dose 2mg. Repeat at 5min intervals to a max total dose of 10mg. PEDI: 0.1mg/kg IV/IO/IM/SQ every 2min as needed. MAX TOTAL dose of 2mg. If no response in 10min administer an additional 0.1mg/kg.

Diazepam (Valium)

A

Dosage: A: Seizure Activity: 5-10mg IV q 10-15min PRN (5mg over 5min)(Max Dose 30mg). Premedication for cardioversion 5-15 mg IV over 5=10min prior to cardioversion. PEDI: seizure activity: 0.2mg-0.5mg/kg slow IV q 2-5min up to 5mg (Max dose 10mg/kg). Rectal Diazepami: 0.5mg/kg via 2” rectal catheter and flush with 2-3mL air after admin.

Flumazenil (Romazicon)

A

Dosage: A: Initial dose 0.2mg IV/IO over 15 seconds. Second dose: 0.3mg IV/IO over 30 seconds. If no response, repeat once every min until adequate response or total of 3mg is given. PEDI: Not recommended.

A

Note: When given IV/IO, must be diluted with equal volume of sterile water or NS. Dosage: A: 2-4mg slow IM/IV at 2mg/min; may be repeated in 15-20min. Max dose of 8mg. for Sedation: 0.05mg/kg up to 4mg IM. PEDI: 0.05-020 mg/kg slow IV/IO/IM over 2min. May be repeated once in 5-20min. MAX dose of 0.2mg/kg
