mainly constipation is the cause so treat that with GTN diltiazen and ligocain for sympotomatic relife warn the patient that it can cause hypotension so they should line down after application

what is important to do for fissures and haemorrhoids
A
check for cancer with sigmoidscopy for perianal examination under anaesthetic if indicated

extermy sore drain or if they are septic treat with antibiotics
