- These drugs have PAE but you don’t want to keep them too low for too long

Enterococcal endocarditis Burns Renal fialure Osteomyelitis Meningitis Pregnancy Cf Febrile neutropenia
CrCl + Tau (hrs
Greater than 60 = 24h
40-50 = 36 hrs
20-39 = 48 hrs
Less than 20 = serial levels to determine next dose time
Draw concentration is what pts?
Critically ill/comorbidities Renal function not normal Long course (adults >3 d or children > 5 d)
Follow up concentration in:
extended duration
Scr up or down significantly (~20% change)
Aggressive dosing
Unexpected results
