How does alpha 1 agonist work?
A1–>vasoconstriction–> increase BP, decrease bleeding, mydriasis, decrease in nasal congestion
Indication for alpha 1
Orthostatic hypotension WL suppression Injection for priapism Ophthalmic for mydriasis in eye exam Glaucoma Decongestant Stress Incontinence
S/E/ A1
syncope, tachycardia, HTN
Caution A1
HTN hx
How does alpha 1 antagonist work?
block A1–> vasodilation–> relax prostate and bladder neck
Indication alpha 1 antagonist
BPH with HTN
S/E alpha 1 antagonist
orthostatic hypotension, dizziness, reflex tachycardia, nasal congestion, sexual dysfunction
How does A2 agonist work?
stimulate central A2–> decrease SNS
Indication A2 agonist
HTN
S/E A2 agonist
dry mouth, sedation, fatigue, postural hypotension
Methyldopa S/E
autoimmune hemolytic anemia
How does non-selective alpha antagonist work?
block A1 and A2–> antihypertensive b/c vasodilation
Indication non-selective alpha antagonist
pheochromocytoma and epic pen reversal
S/E non-selective alpha antagonist
orthostatic hypotension, dizziness, reflex tachycardia, nasal congestion, D/N/V, priapism, peptic ulcer, MI, stroke, arrhythmia
How does beta 1 agonist work?
positive inotrope, stimulate kidney and heart, increase renin release and renal BF
Indication B1 agonist
severe CHF, stress test
S/E B1
tachycardia
How does beta 2 agonist work?
stimulate B2–> bronchodilator and uterine relaxation
Indication B2
COPD/asthma
delay premature labor
priapism
Patho Beta Block
Block beta–>decrease CO, HR and contractility; decrease renin secretion; decreasepost-MI ventricular remodeling
Indication BB
HTN Class 2 tachyarrhythmia HF Angina Acute MI migraine prophylaxis thyrotoxicosis with palpitation acute angle glaucoma benign essential tremor
Caution with BB
DM b/c mask hypoglycemia Asthma/COPD (nonselective agents) 2nd/3rd Heart block Cocaine induced MI decompensated HF worsens PVD and Raynaud
How does A1 antagonist help cholesterol
increase HDL and lower LDL
Methyldopa cannot be used with
