What are three intubation criteria to consider?
1) Patent Airway
2) Failure to oxygenate or ventilate
3) Expected clinical course
What makes a patient an RSI candidate?
1) Requires sedation
2) Do not anticipate a difficult airway
3) You expect to be able to ventilate
Entire goal is to prevent regurgitation
What is the induction dose for etomidate?
0.1-0.3mg/kg
How long does etomidate sedate for?
100 seconds / 0.1mg/kg
If you are giving 0.3mg/kg dose you will have ~300 seconds of sedation which is ~5min.
What is the most hemodynamically stable induction agent?
Etomidate. Will not effect rate or pressure.
What is the induction dose for ketamine?
0.5-2.0mg/kg
Who should you not give Succinylcholine to?
1) Hyperkalemic patients
2) Narrow angle glaucoma
3) Children < 8yo
4) Open/penetrating eye injury
5) Hx of malignant hyperthermia
6) Major burns or crush injuries
7) Musculoskeletal / Neuro disorders
What are the signs of malignant hyperthermia?
1) Increasing EtCO2 that will not correct
2) Tachycardia, arythemias, muscle rigidity, increasing temp, DIC
How of you treat malignant hyperthermia?
1) Increase vent F to blow of CO2 and maximize FiO2
2) Dantrolene 2.5mg/kg (repeat up to 10mg/kg) or 1x vial of Ryanodex
3) Aggressively cool
What is the duration time of Rocuronium?
30-120 minutes
After paralytic induction, how often and what dose should you give to maintain paralysis?
q40 minutes and 1/4 of the induction dose
