2 antibiotics + acid reducer (PPI)
- PPI (-prazole)
- Clarithromycin
- Amoxicillin or Metronidazole
When would we consider quadruple therapy for H. Pylori?
community resistance or recurring H. Pylori inf.
What does Quadruple Therapy for H. Pylori entail?
–A PPI (‘-prazole’)
–Metronidazole
–Amoxicillin(Tetracycline; esp. for PCN allergy)
–Bismuth subsalicylate
•Can substitute Clarithromycin for bismuth
•Can substitute Levofloxacin for failed eradication
Dosing for H. Pylori eradication regimen?
PPI BID x14 days (longer if ulcer is unresolved)
Abx QID x14 days
What would you prescribe for treatment of Clostridiodes difficile?
Vancomycin (ORAL)
alternative: Fidaxomicin
What would you prescribe a very sick pt in ICU with C. diff infection?
IV Metronidazole (for severe disease) + oral Vanco.
