when to adjust eliquis afib/stroke dose?
Age ≥80 years
body weight ≤60 kg
serum creatinine ≥1.5 mg/dL
BP goal for altaplase
under 185/110 before giving it, maintained at 180/105 after for at least 24hr
altaplase contraindicatons
internal bleeding
recent stroke within 3 months
LMWH within 24hr
DOAC/direct thrombin inhibitor within 48 hour
warfarin with INR 1.7
alteplace dosing
0.9 mg/kg (actual body weight), maximum dose 90 mg
10% given as IV bolus over one minute, infuse the rest over 60 minutes
Tenecteplase dosing
0.25 mg/kg (actual body weight), maximum dose 25 mg
Entire dose given as IV bolus over 5 to 10 seconds
for STEMI, PCI is preferred to be preformed within
90 min

reduced: 2.5mg BID
xarelto afib dose
20mg daily
warfarin factors
factors II (half-life 42 to 72 hours),
VII (half-life 4 to 6 hours),
IX, and X (half-life 27 to 48 hours)
