Pharmacology of adrenaline

A

Naturally occurring catecholamine which acts on A1, B1, B2 receptions.
A1- Peripheral vasoconstriction
B1- Increase in HR and force on myocardial contraction = increase in irritability of the ventricles.
B2- bronchodilation

Indications for adrenaline

A
  • Cardiac arrest
  • Anaphylaxis / severe allergic reaction
  • Severe-life threatening brochospasm/ silent chest.
  • Shock unresponsive to adequate fluid resus.
  • Croup.

Dosage of adrenaline

A

Cardiac arrest:
IV- 1mg every 3-5 mins, no max dose

Analphylaxis
IM- 300microg Epipen
IM- 500microgs, every 5 mins, no max dose

Severe life threatening bronchospasm/ silent chest:
IM- 500microg every 5 mins no max

Croup:
Neb- 5mg single dose only
