adrenal glands Flashcards
adrenal medulla
part of autonomic nervous system “fight or flight”
catecholamines: increase metabolism and glucose
epi & nor-epi
pheochromocytoma
deadly benign tumor of the adrenal medulla
pheochromocytoma manifestations
hypertension
headache
hyperhydrosis
hypermetabolism
hyperglycemia
testing for pheochromocytoma
24 hour urine for catecholamines
plasma catecholamine
clonidine suppression test (neg=pheo)
MRI/CT
treatment for pheochromocytoma
alpha blocker “-azosin”
beta blocker
calcium channel blocker
high sodium diet
pheochromocytoma nursing considerations
continuous telemetry
watch EKG, vitals, glucose
possible steroids needed
adrenal cortex
glucocorticoids (cortisol): ^ glucose & metabolism
mineralocorticoids (aldosterone): sodium reabsorption
androgens
addison disease
adrenocortical insufficiency- genetic condition
testing for addison disease
decreased cortisol, increased ATCH
ACTH stimulation test
addisonian crisis
dehydration
profound fatigue
renal shutdown
pigmentation issue
treatment for addisonian crisis
steroids
IV hydrocortisone
cushing’s syndrome
excessive adrenocortical activity
cushing’s syndrome manifestations
buffalo hump
hurtuism
decreased libido
more masculine
testing for cushing’s syndrome
24 hour cortisol urine: if 3x higher= cushings
not normal cortisol fluctuation
dexamethasone suppression: if >5= cushings
decreased ACTH, increased cortisol
treatment for cushing’s syndrome
take away the steroids!!
adrenal enzyme inhibitors metyrapone
