What is an “order”
Plan of care or any change in action
*A new order must be written for any alteration
When are verbal orders okay?
In emergent or urgent situations
What is the acronym for initial admission orders?

What does AD+ CAVA+ stand for?
A: admit/transfer
D: diagnosis

What does DIMPLS+ O stand for?
D: diet
IV/I: Interventions

What should go under admit?
- Attending HCP name
- Unit/Floor
- If PCP is not the same as the attending, part of orders should be notified to them
What should go under Diagnosis
- Diagnosis that caused the patient to be admitted (primary)
- Any other diagnosis that the patient currently has (comorbidities- secondary)
What goes under condition?
- General condition of patient at the time of admission
- Code status
EX: stable, full code
What goes under the activity?
- Restrictions on the patients activity
*bed rest
*Bathroom privileges
*Ambulation
What goes under vital signs?
- Which vital sign+frequency (how often they are checked)
- Parameters of vitals
*Notify is under or above a certain amount
What goes under allergies?
- Medication, food, environmental
- State reaction if known
What goes under diet?
- state any dietary restrictions
What goes under IVs/ Interventions
- Ordering IV fluids
*type of fluid
*any additives
*rate
What does heplock mean?
IV but no fluid flowing
What does KVO mean?
Keep vein open
