What age group is afffected by AC?
40-65
This AC has no known cause
Idiopathic primary adhesive
This AC is due to DM and thyroid issues (hypothyroidism -females)
Secondary systemic
This AC is due to RCT or labral pathology
Secondary intrinsic
This AC is due to cervical disc disease, stroke, or not moving the arm
Secondary extrinsic
How long does AC last?
12-18 mo, but up to 2 years
This stage lasts three mo. Sharp pain at end range, aches at rest. Impingement?
Stage one
What stage? Gradual loss of rom in all planes, limited by pain, no true end feel, and occurs 3-9 mo
Stage 2
What is happening in stage 2 of AC
Aggressive angiogenesis and synovitis
What stage? Lessening of pain, ROM is most limited
Stage 3, mo 9-15
What is happening in stage 3 ac?
Fibrosis
What stage? Significant decrease in pain, gradually improving ROM
Stage IV, mo 15-24
Ruling in AC criteria: 6 things
- Age between 40-65
2 gradual insidious onset with pain and stiffness
3 Rom limited grooming, ADLs and sleep
4 gh PROM most limited in ER and decreases in greater abduction
5 PROM in end range increases pain
6 accessory glides are limited in all directions
Can rule out AC if 6 things
1Normal PROM
2 gh oa on x ray
3ER or IR increase with greater abduction
4 familiar pain is reproduced with palpation
5 ULTT reproduces pain
6 shoulder pain is reproduced with peripheral nerve testing
GH OA can look like AC but will be most prevalent at this age
60 and greater
