What is the difference between adenomyosis and endometriosis?
o Endometriosis = endometrial cells existing outside of the uterus
o Adenomyosis = endometrial cells existing INSIDE the uterine muscular wall
What are the signs and symptoms of adenomyosis?
o Menorrhagia ± dysmenorrhoea
o Chronic pelvic pain (more so during menstruation)
What are the investigations of adenomyosis?
o O/E bimanual -> bulky and sometimes tender ‘boggy’ uterus
o USS -> haemorrhage-filled, distended endometrial glands
§ May show an irregular nodular development (similar to fibroids)
o MRI pelvis (GOLD)
What is the management of adenomyosis?
if amenorrhoea induced, ectopic endometrium becomes quiescent
o DUB/fibroids management pathway
§ 1st line = LNG-IUS ± NSAIDs
o Hysterectomy is the only definitive treatment
