Decreased urine output, altered mental status

What would be the vital signs? We can explain blood pressure being normal?
A
Patient would have tachycardia and bradycardia however if taking anti-hypertensive blood pressure may be normalized

What is your approach for this particular patient With decreased urine output and altered mental status?
A
Decreased urine output it may be secondary to acute renal failure (dehydration, pre-renal). Intrinsic Reno cars from diabetes. How’s Reno obstruction from BPH. The causes of altered mental status may include DKA hypoglycemia hypertensive crisis, stroke, etc.
