Who is most affected ALL?
Children <6 y/o
What is the pathology?
Overgrowth of lymphoid progenitor leading to uncontrolled differentiation into B cells that replace normal B cells
What may be the cause if the ALL is T cells?
Thymoma
Typical presentation?
” 4 y/o presents with tiredness, low grade fever, dyspnoea and bruising. O/e he has HSM. “
What are 3 main symptoms?
HSM
Generalised lymphadenopathy
Fever
Thrombocytopaenia
What is the character the lymphadenopathy?
Freely movable, soft
They are not cancerous they are reactive
Investigation?
FBC
Blood smear
Bone marrow biopsy
What may be seen on FBC in ALL?
Anaemia
Raised WCC
Thrombocytopenia
What type of anaemia is seen in ALL?
Normocytic, normochromic anaemia
What is seen on bone marrow biopsy?
> 20 % lymphoid blasts
DDx?
AML
How do you differentiate between AML and ALL?
Very hard
AML low WCC
Needs bone marrow bopsy
Treatment ALL>
Chemotherapy
HSCT
Treatment of tumour lysis syndrome?
Sodium bicarb
Rehydrate
Allopurinol
