- signs of hypovolemic shock due to hemorrhage
- feeling of impending doom
- melena and hematemesis with upper GI
- hematochezia and black vomit with lower GI
what are common sites for bleeding with upper GI?
esophageal varices and PUD
what medications help treat upper GI problems?
PPIs, IV fluids, antibiotics for H.pylori, and sucralfate
what are common sites for bleeding with lower GI problems?
- polyps
- diverticular disease
- colorectal cancer
- ulcerative colitis/ crohn’s
what diagnostic testing can help check for upper GI problem and what prepocedure cautions should the patient take?
EGD is a diagnostic test for upper GI probems and patient must be NPO 24 hours before due to risk of aspiration
what tests can diagnose lower GI problems?
colonscopy and endoscopy
what labs are ordered for an acute GI hemorrhage?
- CBC with H&H
- Type and cross
- H.pylori to determine PUD
- stool for blood
- BUN/Crt for perfusion status
what are priority interventions to help treat an acute GI hemorrhage?
- priority is to see patient with changes in LOC
- NG tube decompression to get blood out of esophagus if this is an upper GI bleed
- stop the bleeding
- 2 large IVs and administer fluids or blood
what are treatment options for a lower GI bleed?
- treat the hypovolemic shock

what is the major complication with an acute GI hemorrhage and what are nursing care points here?
HYPOVOLEMIC SHOCK
- note estimated blood loss
- fluid resuscitation
- give blood products
- patient will go to ICU
