periumbilical pain moves to RIF. anorexia, vomiting, constipation
general signs
tachycardia, fever, furred tongue, lying still, coughing hurts, foetor +- flushing, shallow breaths
signs in RIF
guarding, rebound and percussion tenderness, PR painful on R side
what is Rosvings sign
pain in RIF > LIF when LIF is pressed
what is psoas sign
pain on extension of hip due to retrocaecal appendix
what is cope sign
pain on flexion and internal rotation R hip if appendix close to obturator internus
investigations
incr CRP, neutrophil leucocytosis, USS, CT
what would be the presentation if the inflamed appendix was retrocaecal/retroperitoneal
flank or RUQ pain
treatment
appendicectomy. antibiotics- metronidazole + cefuroxime
DDX
ectopic, UTI, mesenteric adenitis, cystitis, cholecystitis, diverticulitis, salpingitis/PID, dysmenorrhoea, crohns, perforated ulcer, food poisoning
complications
perforation, appendix mass, appendix abscess
