Hamilton Fire Department Policy on report requirements:

A
  • Fire reports and forms will be filled out with all pertinent information (if available) in a timely manner by the officer in charge of the incident, or the officer of the station unless otherwise stated
  • All reports are completed in FDM-RMS
  • All reports shall be completed in metric
  • For the requirements of all reports, the incident commander will be the officer in charge when the situation was declared under control

A

Structure type, construction, size, occupancy, content
Protection Systems and system status on arrival/departure
Name/Address/Contact info/Tenant responsible parties/witnesses, relationship to incident
insurance, damage/loss to what (building/contents)

Police notified/run number/when did they arrive?
OFM Notified/run number/name of investigator?
any other relevant information that may arise during the incident

A
