- Exercise Participation Health Screening
Health Screening Tool/Physical Activity Readiness Questionnaire for Everyone/Health History Questionnaire
- Health screening tool to identify level of risk
- Indicates if members should seek consultation from health care professional prior to physical activity
- Reviewed by qualified staff & retained for ≥ 1 year
- Medical clearance is recommended if CV, metabolic, renal disease, or other medical concerns identified
- Renew annually or with significant change in health status
- Member Orientation, Education, and Supervision
Facilities operators offer new or prospective members orientation to new facility
Provide members who engage in physical activity program assistance/guidance with their efforts
- Emergency Planning and Policies
Written policies & procedures, reviewed regularly and rehearsed twice annually
Conduct safety audit to routinely inspect all areas of facility & remove hazards
Written communication to share OSHA (Occupational Safety & Health Administration) guidelines on handling of potentially hazardous materials, including bodily fluids
Follow automated external defibrillator AED requirements and have a written policy & procedure to include public access defibrillator program
- AED location should facilitate 3-5 minute or less response time from collapse to defibrillation
- Health/Fitness Facility Professional Staff and Independent Contractors
Health/fitness professionals & staff shall have appropriate levels of professional education, work experience, and/or certification
- must have current automated external defibrillation and cardiopulmonary resuscitation (AED and CPR) certification
- Health/Fitness Facility Operating Practices
Electronic or manual operational systems in place
Temperature settings are appropriate & equipment is maintained
Warning signage must be in place
Provide evidence of water-chemistry safety mandated by state & local codes & regulations
Properly secure physical and electronic data to protect against a data breach and the release of their personal information

Provide evidence of compliance with state & local laws & regulations
Require completion of appropriate paperwork (waiver, an authorization for emergency medical care, and a release)
Provide names of persons who can pick up each child
Maintain records of date/time of checkout, drop-off, & adult picking up child (verify identity)
- Health/Fitness Facility Design and Construction
Adhere to the standards of building design, as detailed in the Americans With Disabilities Act (ADA).
Follow all applicable federal, state, and local building codes.
Provide adequate clearance to the side and at the rear of all types of continuous-motion exercise equipment.

Aquatic and pool facilities must provide the proper safety equipment

Policy on removal of broken/damaged equipment
Equipment to accommodate physical limitations
- Signage in Health/Fitness Facilities
Must post:
Proper caution, danger, and warning signage
Emergency and safety signage pertaining to fire and related emergency situations
Signage indicating the location of any AED and first-aid kits
ADA and OSHA signage
Cautionary, danger, and warning signage must have required signal icon, signal word, signal color, and layout, as specified in ASTM F1749
