Initial management (4 things)
Aspirin 300mg
O2 if sats <94%
Morphine if severe pain
Nitrates - avoid if BP low
STEMI - step 1
Aspirin 300 mg
STEMI - PCI in 120 mins? Yes (4 things)
Prasugrel
Radial access
Give unfractionated heparin
Drug eluting stents
STEMI - PCI in 120 mins? No (2 things)
Antithrombin
Then ticagrelor
STEMI - persistent ECG changes 90 mins after fibrinolysis
PCI
STEMI - patient high bleed risk
Swap prasugrel for ticagrelor
Swap ticagrelor for clopidogrel
STEMI - patient on oral anticoagulant
Swap prasugrel for clopidogrel
NSTEMI - step 1
Aspirin 300 mg
Fondaparinux if not immediate PCI
NSTEMI - step 2
Calculate GRACE score
NSTEMI - <3% (low risk)
Ticagrelor
NSTEMI - >3% (intermediate/high risk)
PCI within 72 hrs
