Where is GH produced?
Anterior pituitary
Most common cause of acromegaly?
Unregulated GH secretion by a pituitary adenoma
Rarer cause of acromegaly?
Secondary to lung or pancreatic cancer, that secretes ectopic GHRH or GH
What type of vision loss occurs as a result of a pituitary adenoma?
Bitemporal hemianopia
How does Acromegaly present?
- Headaches
- Visual field defect
- Large nose, tongue, hands, feet
- Large protruding jaw (”prognathism”)
- Profuse sweating
- Development of new skin tags
List 2 complications of Acromegaly
- Hypertrophic heart
- Hypertension
- Type 2 diabetes
- Colorectal cancer
Investigations for suspected acromegaly
Highlight first line
- IGF-1 (first line)
- OGTT while measuring GH
- MRI brain
- Refer to ophthalmology for visual field testing
Explain the OGTT + GH for acromegaly
High glucose normally suppresses GH
(+) test, GH remains elevated
Definitive treatment for acromegaly?
Trans-sphenoidal surgery if pituitary adenoma
If caused by ectopic hormones, surgery for specific cancer
List an example of a medication which can be used to block GH
Somatostatin analogues eg. ocreotide
