List the steps to take when you have:
DAMAGE TO APPARATUS (ACCIDENT INVOLVEMENT INJURED/NON-INJURED)
Take apparatus out of service
Notify Captain
Contact Alarm Rm. to create incident number
Contact BC (No answer contact WDC)
Contact GPD 623-930-3000 or other city agency accident occurred in
Contact Don Jessee 623-696-9159 (leave message)
Go to FD website fill out COG Injured Person/Accident
Report submit to Captain
(** DO NOT FILL OUT LOST/DAMAGED EQUIPMENT FORM**)
Fill out RFS on GFD website
