A

Gastrointestinal (GI):

  1. • Abdominal pain (acute/chronic)
  2. • Indigestion
  3. • Nausea/Vomiting
  4. • Decreased appetite
  5. • Difficulty swallowing (dysphagia)
  6. • Pain swallowing (odynophagia)
  7. • Fill up quickly (Early satiety)
  8. • Change in bowel function (+/-)
  9. • Diarrhoea/Constipation/incontinence
  10. • Jaundice
  11. • Blood in/around stools
  12. • Black sticky faeces (melaena)

Focussed Abdo History (GU)

A

Genitourinary (GU):

  1. • Lower back or lower abdo pain (flank/suprapubic)
  2. • Difficulty passing urine (dysuria)
  3. • Urgency/frequency of urination
  4. • Increased volume of urine (polyruria)
  5. • Need to go in the night (nocturia)
  6. • Blood in urine (haematuria)
  7. • Sexual history/concerns
  8. • STD history
  9. • Genital sores or discharge

Men:

  1. • Hesitancy/decreased stream
  2. • Testicular pain/masses

Women:

  1. • Obstetric history
  2. • Abnormal vaginal bleeding
  3. • ?pregnancy
  4. • Last period

4 regions - Increased, decreased, absent (only > 5mins)

Abdominal Assessment (Percussion)

A

9 regions:

  • Tympany = gas
  • Dullness = solid or fluid
  • Normal = Would expect a mix
  • If dull on sides and tympani in centre – would perform ascites test.

Ascites test:

  1. Fluid Wave test: Pt presses into centre of abdo. Feel on one side and gently flick contralateral side – will feel wave on contralateral side if fluid
  2. Shifting dullness: Central tymany, lateral dullness - Level of dullness shifts when patient turns onto side

Abdominal Assessment (Palpation)

A

Light palpation – to identify abdo tenderness, muscular resistance, suerfical organs and masses

Deep palpation - Identify any pain or masses noting:

Location, size, shape, consistency, tenderness, pulsation

Costovertebral tenderness - Renal tenderness

** rebound tenderness = peritoneal sign
