Preventif: 5000 ui x2/j = 0,2ml x2 par J
Curatif : bolus 80UI/kg puis 500ui/kg/j en 3 injection sous cut ou ivseou 18ui/kg/H

HBPM
Preventif: 0,4ml/j 4000ui enoxaparine
curatif: enoxaparine 100ui/kg/j x2 ou 0,01ml/kgx2

Si tinzaparine : 175 ui/kg curatif
