A

Static risks are predictable and all remain constant over a period of time. Yes they can be planned.

What are dynamic risks? How do you manage these?

A

Maybe difficult to predict/occur because of sudden changes. They are hard to plan for and controls. May need to be implemented “ On the run”.- By using TENR And also doing a risk assessment, HNS should be included in any TENR and AFCO

A

Critical risk as death or life-threatening injury

How should incidents of near misses be reported? What does it not include?

A

My Police by persons involved. If they can’t a supervisor should. Debrief or review of what happened and what can be learned to avoid it happening again. Not blame. Must be viewed by supervisor

What is a near miss? What does it Not include?

A

I miss incident that could’ve caused harm but didn’t “ Damn that was close” event.
It includes “Loss only” Incident with damage was to Police property, Minor car crash and no people harmed. And near miss as an opportunity to prevent a future injury before someone has hurt.
Doesn’t include
- An unsafe condition - Patrol car with bald tyre and Police know about it
- An unsafe act - Police take car out knowing it has a bald tyre.

What is a critical incident? What does it include?

A
  • Incident involving death/life-threatening injury to a person during Police activity or to a person and Police custody or under care of Police
  • Please discharging a firearm at a person intentionally whether or not death/injury
  • Accidental discharge results of death/injury

What happens following a critical incident?

A

Internal/external investigations/reviews may take place.Police critical incident investigation is the lead investigation with a range of objectives to
- Determine the circumstances of the incident
- Gather and preserve evidence
- Establish with the force used was legally justified and whether any person is criminally liable for actions taken. Welfare needs and reputation risks are managed.

A number of reviews/investigations may take place
- Play some investigation into the critical incident to establish the facts of what occurred and determine whether the police employees or any other persons are criminally liable for actions taken
- IPCA investigation
- A policy,practice and procedure review (PPP)
- A coroners enquiry at the person has died
- A WorkSafe investigation H&S Act 2015
- An internal Place health and safety review
- And employment investigation may be carried out separately if potential breach of the code of conduct was identified.

All information generated by the police investigation may be shared with other agencies
