What are unique of platelet morphology compared to other species?

A

Mammalian platelets do not have a nucleus, therefore, not a true cell. They are cytoplasmic fragments of megakaryocytes
Bird, reptile and amphibian platelets contain a nucleus and are true cells

A

Vary in size/shape
usually sm, round, w/ distinct pink to purple granular cluster in the center while periphery is pale blue
May have long, thread-like processes extending out in all directions

A

look similar to canine platelets
cigar-shaped and giant platelets (sim size to RBCs) more common in cats and MUST BE REPORTED - bc it could look like WBC’s
Inc tendency for platelets to clump

What is unqieu about equine platelets

A

Indistinct, often described as “ghostly” in appearance - very pale and may be difficult to see
Granules are difficult to see and are few in number
Platelet counts are often low in healthy animals

What are unique about bovine platelets

A

Granules are prominent
platelets usually sm but giant forms do occur
pseudopod-like projections may be present
individual platelets often appear superimposed on red cells and must be differentiated from parasites
platelet counts are often high in healthy animals

What should you do if you are having a hard time finding platelets?

A

Scan feathered-edge of the smear to check for aggregates/clumping
Clumping is common, they are larger entities and are “sticky”
when smear is made, clumps get pushed to feathered edge
Clumping is to be reported
Sometimes clumps are so numerous and/or large that platelet counts are not accurate

What are giant platelets?

A

Megathrombocytes
Can be bigger than RBC’s
Giant, cigar shaped and other oddly shaped platelets should be noted on report
giant platelets may affect total RBC and platelet counts generated by automated analyzers

A

Epistaxis, petechia, or unexplained hemorrhages - used to eval for evidence of thrombopathia

What is platelet enumeration?

A

most often don using automated analyzers
can be very inaccurate due to platelet clumping, similarity in size to RBCs - problem in cats
Need to use a fresh EDTA blood sample and make sure there are no clots

How do you estimate platelets?

A

Platelet estimate is performed by examining the blood smear in the monolayer region - should always be done as part of QC for your machines
Count a min of 5 oil immersion fields of view - normal patient will have btw 7-10 platelets/hpf

What is the formula for estimating platelets?

A

of platelets in 5 oil immersion fields X 20 = xx X 10^9 platelets/L

No decimal plates

At what point should we perform an actual platelet count?

A

When estimates are lower than
 Canine: <200 x 10⁹/L
 Feline: <300 x 10⁹/L
 Large Animals: <100 x10⁹/L

What are some platelet indices?

A

Performed by many automated analyzers
Include
Mean platelet volume (MPV)
Plateletcri (PCT)
Platelet distribution width (PDW)
Usefulness of platelet indices is not well documented for vet species

What is mean platelet volume?

A

Average platelet size counted by analyzer
Mathmatically calculated, measured in femtoliters (fL), not useful for reline or Cavalier King Charles Spaniels, platelets will swell with EDTA so run samples and make slides as soon as possible to minimize artifacts

What does a high VS low MPV value in a dog tell?

A

High MPV in dog tells us there is an adequate bone marrow response
Low or normal MPV in dogs is not significant and doesn’t give us any additional information
Anticoagulants can affect results

What is plateletcri/thrombocrit (PCT)

A

is percentage of total blood volume made up of platelets - similar to PCV
Usefulness is unknown

A

Assesses the variation in platelet size
Often reported using a histogram
usefulness is limited

A

Used to assess for evidence of thrombopathia
automated analyzers in the clinic test the ability of the platelets to aggregate and secrete platelet factors - idexx’s coag Dx
Manual test - buccal mucosal bleeding time

What are some additional evualtions for platelets

A

referral labs perform more specific platelet evaluation
Some of their tests include - reticulated platelet count (newly released platelets, equated to RBC reticulocytes in that they both indicate a bone marrow response
Antiplatelet antibody assays - evaluate for the presence of antibodies on the surface of the platelets
