A

Naevi are ‘ocular benign pigmented lesions’.
They are the most common conjunctival tumour that can be patchy in colour with haloing. Form in pubescent years due to skin stretching and increased blood supply. They grow very slowly. They are either flat or minorly elevated and unilateral while close to the limbus. They commonly only have one or two feeder vessels and cysts are present in 70%.

A

Naevi are classifed based on depth or cell type/appearance:
1. junctional ones are found in the epithelial basal layer/stroma junction
2. intradermal ones are found in the stroma
3. compound ones are both 1 and 2

A

Congenital melanocytosis are benign pigmented lesions.
Defined by increased colour of deep tissues due to excess melanosomes, particularly the sclera or skin of lids. It’s called congenital melanosis if it affects the eye alone, it’s called Naevus of Ota if it affects the eye and skin. About 10% have associated glaucoma and 2-4% an develop into melanoma. Can change or appear at puberty.

A

Primary acquired melanosis is an ocular premalignant pigmented lesion.

Affects middle aged, fair skinned people and looks like a unilateral, diffuse, pigmented lesion. Can be found anywhere on conjunctiva, but especially on the lid margins. They are quite flat. Based on lateral proliferation of melanocytic cells (like CIN), can move since they affect the conjunctiva, and have minimal feeder vessels.

A

Melanoma are ocular malignant pigmented lesions.
75% develop from primary acquired melanosis. They are invasive which means they are not motile over sclera and often invade the cornea. They grow quickly and have dense coloration. They have irregular boundaries with small haloes. They have lots of feeder vessels and can also have hemorrhages. They are raised.
