Myocardial infarction -> impaired contractility -> dec cardiac output -> inc end diastolic volume -> inc end diastolic pressure in L ventricle -> inc L atrial pressure -> inc pulmonary pressure -> pulmonary hypertension -> pulmonary congestion -> shortness of breath
