6: Lab Values Flashcards
Calculation estimate for Hb based on hematocrit
Hb = hematocrit / 3
Eight values obtained from a BMP
- Na
- K
- Cl
- Total CO2
- BUN
- Creatinine
- Glu
- Anion gap
CMP: six things added to a BMP
- AST/ALT
- Bilirubin
- Alkaline phosphatase
- Ca
- Albumin
- Total protein
Anion gap definition
Concentration of unmeasured anions in blood (protein, phosphate, sulfate, and organic acids)
Formula for Anion gap
AG = Na - (Cl + HCO3)
In outpt settings, testing for thyroid dz starts with testing TSH. What is the next step with a normal, high, and low TSH?
- Normal TSH: no further testing needed
- High TSH: free T4 added to determine degree of hypothyroid
- Low TSH: free T4 and T3 added to determine degree of hyperthyroid
Normal range of TSAT
25-45%
Level of Hb that qualifies as anemia in women and men
<12 in women
<14 in men
Neuro sx suggestive of Vitamin B12 deficiency anemia
Decreased vibratory and positional senses, ataxia, parasthesia, confusion, dementia, pearly gray hair at an early age
Signs of profound IDA
Koiloncychia (spoon nails), gloss ITIMs, dysphagia, PICA
What treatment will improve all anemias with hemodynamic instability
RBC transfusion
Treatment for IDA
Supplemental Iron - can be oral but may require parenteral infusions
What certain conditions require treating IDA with parenteral iron instead of oral
Malapsorptive conditions, malignancy, CKD, HF, significant chronic blood loss
B12 deficiency treatment
IM B12 shots, 1000 mcg daily for 7 days - >weekly for 4 weeks -> monthly for life
Folate deficiency treatment
Oral folate, 1mg/day until corrected
