What are the general steps to an OHE?
- Locate one of the uterine horns
- Break down suspensory ligament to exteriorize the ovary
- Clamp proper ligament to keep traction on it
- Clamp ovarian vessels with Rochester carmalts proximal to the ovary
- Ligate and cut ovarian blood vessels within ovarian pedicle
- Ligate and resect uterine body and blood vessels just above the cervix
What is the approach to beginning a feline OHE?
- Ventral midline
- Divide abdomen into 3rds from umbilicus to pubis
- Incision centered on middle 3rd of caudal abdomen
What size blade should be used for the initial incision and for what layers?
10 - skin and into the SQ
What should Metzenbaum scissors be used for when entering the abdomen?
Pushing SQ tissue off the body wall
How should the linea alba be opened?
Tent up and make a stab incision (blade facing up), extend the incision carefully
What suture should be used to ligate the ovarian pedicle?
PDS 2-0 or 3-0
What two ligatures should be used on the ovarian pedicle?
- Miller’s Knot (closest to body) in crushed area of 1st forceps
- Transfixation or encircling ligature in crushed area of 2nd forceps
Where do you cut to transect the ovarian pedicle and what is used to do this?
Cut between 2nd carmalt and the ovary;
Use Metzenbaums or scalpel blade
What should you do before you let go of the pedicle?
Hold with thumb forceps and evaluate hemostasis
After the ovary has been transected, what is done with the broad ligament?
It is torn to the uterine bifurcation
