4 in 10 of men in their 50s
What causes it?
Hyperplasia of stromal and glandular cells
Thought to be mediated by dihydrotestosterone
Compression of the urethra –> obstruction of bladder outlet
What risk factors are there?
Obesity
Diabetes
FH
How does it present?
LUTS - hesitancy / urgency Straining Prolonged micturition Frequency Nocturia
What signs may the patient have on examination?
Palpable enlarged prostate on DRE
Smooth surface, non-tender, loss of median furrow
What other conditions might present similarly?
Malignancy
UTI

How would you investigate?
DRE
PSA blood test
Urinalysis (glucose)
What treatments are there?
Depends on symptoms Alter fluid intake, bladder training Medicines - alpha blockers, anticholinergics, diuretics Catheterisation TURP
