Cell surface: pili, LOS porin proteins
N. meningitidis xteristics
Glucose + maltose. Serotypes. Capsule.
N. gonorrhea xteristics
Glucose. No capsule. Strains. Opa protein (attachment, dual variation). Facultative intracellular.
N. gonorrhea virulence factors
Pili, LOS, opa protein, porins, IgA protease, B lactamase
At risk population for disseminated N. gonorrhea
People with late complement deficiency
N. gonorrhea infection
In genital tract, rectum, conjunctivitis, pharyngitis
N. gonorrhea symptoms
Men: urethritis, dysuria, pus
Women: asympotomatic OR cervicitis, dysuria, ↑ discharge. Can lead to salpigitis, PID, infertility
Disseminated N. gonorrhea symptoms
Fever, migratory arthralgias, purulent arthritis, pustular rash on extremities
N. gonorrhea treatment and prevention
Ceftriaxone + azithromycin (chlamidya co-infection tx). No vaccine.
N. meningitidis transmission
Respiratory droplets and close contacts. Late complement deficiency. Dry and cold months. Affects mostly kids. #2 cause CAMeningitis in adults.
N. meningitidis virulence factors
Polysaccharide capsule LOS!!!, IgA protease. MO can cross BBB.
N. meningitidis clinical syndromes
meningitis, meningitis with meningococcemia (blood infection), or
meningococcemia without clinical evidence of meningitis.
Meningitis symptoms
Sudden onset fever, headache, stiff neck, photophobia, myalgias, N/V
Meningitis treatment and prevention
Penicillin or ceftriaxone. Abx prophylaxis: rifampicin, cipro, ceftriaxone. Vaccine: polysacc capsule vaccine, polyvalent vax (A,C,Y,W), Serogroup B vaccines.
