What is the RVT’s role in the sx room?
maintain sterility and assist surgeon - retraction of tissue, lavage sx field w/ steril fuild, running suture
What are the two sources of contamination?
Endogenous: from patient itself
Exogenous: from sx team and/or sx enviro
Before a surgery, what should the technician do in regards to selfcare?
Eat food, drink liquids, visit the restroom, groom fingernails, avoid cologne and perfume
What are some common ways RVT’s can avoid weakness or nausea
avoid locking the knee joints while assisting - slowly shift weight from leg to leg
eat b4 scrubbing in
What is the proper attire in the sx area?
Reduce shedding of microorganisms - freshly laundered scrub suits, head covers, masks, perhaps shoe covers or designated shoes for surgery
Follow written and posted dress code
Follow provided guidelines for general personal cleanliness
What are good house keeping rules about your scrub suit?
Never wear street clothes in sx
launder scrubs in hospital laundry
Don’t wear scrubs from work-home
Scrub top should be short-sleeved, fit snug to body, and be tucked into pants
Clean lab coat over scrubs if stepping out of sx area
What is important about masks and shoes when going into surgery
Masks: worn over mouth/nose, changed frequently, conform to face
Shoes: comfortable and clean, can use shoe covers
What happens if you need to sneeze while in sx?
Do not turn your head!
Step out of sterile field, keep facing forward
What is the surgical hand scrub and hand rub?
sx hand scrub is process of removing microorganisms from nails, hands, arms using mechanical and antiseptic cleaning
Mechanical cleaning: removal of bact, dirt, oils and debris that are loosely attached to the skin using friction
Use of antiseptic inactivates or inhibits the growth of microorganisms on skin, in hair follicles, sebaceous glands and sweat glands
What are some ideal characteristics of scrub solutions?
FDA compliant, broad spectrum, fast acting + effective, non-irritating, persistent effects and cumulative activity
What are the different types of scrub solutions
Chlorhexidine gluconate 4%
iodophor 7.5%
Alcohol 60-90%
parachlorometaxylenol (PCMX)
Brushless scrub
What are the pre-scrubbing guidelines?
clean and thoroughly dry eyeglasses
apply cap and mask
open gown and glovepack
remove rings, watches, jewelry, name tags, pens from scrub top pockets
nails short/clean
Wash hands and forearms up to 2 inch above elbow w/ antimicrobial scrub and running water - dry with a paper towel
What is the steps for a surgical hand scrub?
NAILS: Clean under your nails on both hands under running water using disposable nail-cleaning pick
RINSE: Keep hands above elbows
ANTIMICROBIAL AGENT: apply generous amount of antimicrobial agent to wet hands
HANDS: 1m/hand, brush each fingertip going under fingernails, scrub all 4 sides of fingers, include base of fingers and webbing btw fingers. Scrub lateral sides of the hand, palm and back of your hand to your wrist
ARMS: 1m/arm, scrub forearms from wrist to halfway up to elbow-30s/arm, scrub mid forearm to elbow - 30s/arm
HANDS: drop scrub brush and rub hands together for 1m
RINSE: rinse soap from tips of finger to elbow. Remember > keep hands above elbows!!
STAY STERILE: no touchie touchie during or after surgical scrub
What do you do once you’ve finished the surgical hand scrub
Use hand towel from gown pack to dry your hands and arms b4 gowning up
Start @ your hands > elbows
Do not allow towel to touch your body
Flip your towel after drying your first arm to prevent contam when you are drying your second hand
Why do we glove and gown?
to create a barrier btw sterile and nonsterile areas
open on a dry, flat surface away from other sterile supplies
