4/4- Peripheral speech & language therapy management issues Flashcards
How does somebody get a caseload?
- SLPs @ a school usually “inherit” a caseload and then dismiss and add clients with time
- SLP @ a hospital may “inherit” some ongoing clients and take on daily tasks (i.e.: swallow evals that are new)
- CFY may start out with no clients in clinic and then add new clients as they are referred, or add new clients as they complete assessments and the client qualifies
in terms of materials management–what does the SLP use?
standard materials should be provided by your employer
- paper/pens
- tongue depressors
pen light
tissues
mirror
alcohol/hand sanitizer
gloves
some basic tx. materials
What are basic therapy materials?
- books for all ages
- toys for various ages & developmental stages
- magazines
- newspaper
- hobby books, cook books, etc..
- reinforcers/rewards
- -we need to make sure that they are appropriate for the age range/skill level
In the schools, what therapy materials will you get? and what are some issues you may face?
- you will “inherit” materials. However, you will bring some of your own & you will add as you need materials for specific clients
- photocopies in file folders may be outdated
- the last SLP bought a lot of the materials herself
- tests
- basic tx. kit: figure 7.2
In hospitals, what do materials depend on?
what your case load is
What may your caseload be in the hospitals
- bedside swallow evals
- modified barium swallow evals
- Rehab clinic
- outpatient clinic
In the clinic where will you get materials?
will most likely have a closet of materials either individually or collectively. You will most likely also bring your own materials.
How do we use games in therapy?
- be sure the game is designed to address the goal/objective
- help client remain focused on the communication even being addressed rather than concepts of game
- choose games that have no specific rules to best serve goals
- Make game functional in group sessions and remove competition
- learn to adapt games any way possible to (a) increase the numbers of opportunities for clients to respond (b) eliminate the element of competition (c) decrease wasted time
When scheduling, what are things we need to consider?
nap time, school hours, doctor appointments, your lunch, your bathroom breaks, how many clients, and what type (back-to-back, library/recess/lunchtime) and how much time do they get
In the California public schools, what do they use to determine the presence of an impairment and the amount of time for therapy?
Communication Severity Scale (CSS)
Where are client files kept?
in a folder/file for each client
keep files under lock & key–includes assessment and informed consent data
follow procedure of the work environment
When creating the therapeutic environment, we need to adjust the physical and physiological climate designed for the client’s maximal involvement, how do we develop the context for tx.
- seating arrangement
- room size
- furniture type
- temperature
lighting
noise level
*think of certain clients who may be susceptible to certain colors, lighting, and sounds.
How are treatment plans written?
style may vary but plans are based around goals/objectives
schools=IEP
clinic=SOAP notes/lesson plans
In terms of documentation, what should a chart include
working file or daily chart should include
- service date for tx
- attendance
- goals/objectives
- client performance data
- recommendations for next session
- conversation with caregivers
What do progress notes say?
various styles exist but each ‘note’ documents client’s progress–what is the data based on? ?
