from the caecum
What age is the peak incidence of appendicitis?
10-20 years old
Where is the abdomen most tender in appendicitis?
McBurney’s point
Where is McBurneys point?
one third from ASIS to umbilicus
How does the pain change in appendicitis?
move form central abdo pain, down into the RIF
What is Rovsing’s sign?
Palpation of LIF cause pain in RIF
What finings would you expect on examination?
- Rovsing’s sign
- guarding on palpation
- rebound tenderness
- percussion tenderness
- nausea and vomit
- loss of appetite
(rebound and percussion tenderness suggest peritonitis from ruptured appendix)
How is appendicitis diagnosed?
- clinical presentation and raised inflammatory markers
- CT can confirm diagnosis
- US can exclude ovarian / gyne problem
4 key differential in appendicitis?
- ectopic pregnancy: do serum or urin bHCG
- ovarian cysts (especially if they torsion)
- Meckels diverticulum (2% of population asymptomatic, can bleed, inflame, volvulus, intussusception)
- mesenteric adenitis (usually younger children post tonsillitis or URTI)
Surgery is definitive management of appendicitis, what age can have adult surgeons?
Older than 10 yo can have adult surgeons
