2b. OSCE Approach to Cervical Complaint Flashcards
Landmarks to palpate on the neck

Examination of neck lymph nodes

Examination of the trachea

Examination of the thyroid

Where would you palpate the carotid pulse?

What is central neuropathy?
- nerve entrapment by musculoskeletal or myofascial tissue
- produces parethesias in the area of the distribution of the nerve
- creates sensory dysfunction/pain
- may also decrease muscle strength
- common sites: intervertebral foramen & thoracic outlet
Compression Test & Spurling’s Test

Neck Distraction Test

What is thoracic outlet syndrome?
- results from compression of NV bundle (subclavian a., subclavian v., and brachial plexus) through neck and shouder
- distal pulses may be decreased or absent
- digital cyanosis and ischemia may be present
- some assoc. with sympathetic autonomic dysfunction > UE symptoms
Adson’s test

Roos Test

What should you rule out before performing tests for subarachnoid irritation?
injury to C-spine or cervical cord
Nuchal Rigidity Test

Brudzinski’s Sign

Kernig Sign

axial compression causes C1 fx
Jefferson fracture
flexion and compression injury
Wedge fracture
What accounts for >50% of C-spine injuries?
axial load (force transmitted to bone and disc)
What happens when neck is flexed to 30 degrees?
Cervical lordosis is lost and protective soft tissues are no longer protective
