What is the 2nd line treatment for a patient with COPD with a history of atopy?
Add a LABA and inhaled corticosteroid
What are features of multiple system atrophy?
Parkinsons- tremor, rigidity
Autonomic disturbance e.g. erectile dysfunction, urinary incontinence, postural hypotension
Cerebellar signs
What are features of thyroid storm?
Pyrexia Tachycardia Liver abnormalities e.g. jaundice Confusion N&V Heart failure HTN
What is the treatment of thyroid storm?
Beta blockers and thryoid medication e.g. PTU
What are features of Klinefelter syndrome?
Taller than average Lack of secondary sexual characteristics Small firm testes Infertile Gynaecomastia Elevated LH and FSH but low testosterone
What is a raised GGT indicative of?
Excessive alcohol consumption
What is the initial management of any intracerebral haemorrhage?
Stop any anti coagulents and control BP
What is the 2nd line management of COPD if there are no features of atopy?
If on SAMA switch to SABA
Add LAMA and LABA
What are features of allergic bronchopulmonary aspergillosis?
Bronchoconstriction- wheeze, cough
Bronchiectasis
What is found on investigation of bronchopulmonary aspergillosis?
Eosinophilia
Fitting CXR changes
Raised IgE
What is the management of allergic bronchopulmonary aspergillosis?
Steroids
Itraconazole can be added 2nd line
