Schamroth sign
If clubbing is absent, the window appears between two pointer nails together (small diamond shaped window)
Increased volume of the clubbed digit is primarily due to increased amounts of vascular connective tissue

Right away you take a wide margin excision if you suspect this - asymmetry, irregular border, etc.
