Cholestasis
Decrease in bile flow due to impaired hepatocyte secretion/ obstruction of bile flow thru bile ducts
Two major functions of bile
- Emulsify dietary fat
2. Eliminate bilirubin, excess cholesterol, xenobiotics, and trace metals
What is bilirubin?
Toxic end product of heme degradation
Normal serum bilirubin levels
0.301.2 mg/dL
Two mechanisms that lead to excess unconjugated bilirubin
Excess production + defective conjugation
Unconjugated bilirubin is mostly bound to what?
Albumin in blood
Three major causes of conjugated hyperbilirubinemia (everything else likely unconjugated)
- Canalicular membrane transport deficiency (Dubin-Johnson, Rotor)
- Hepatocellular disease (cirrhosis, hepatitis)
- Impaired bile flow from obstruction or PBC/PSC
Three major causes of cholestasis
Stones, tumors, strictures
What can long-term cholestasis cause? (Five things)
- Biliary cirrhosis (fibrotic gallbladder)
- Sepsis
- Intrahepatic infection
- Ischemia
- Primary hepatolithiasis

- Coliforms
2. Enterococci
