O/I of quadratus lumborum?
O- iliac crest
I- 12th rib and tp of L1-4
Quadratus lumborum fxn?
assist in respiration
What happens if both QL contract? only 1?
Both- extension
1- extension and sidebending ipsilaterally
A sapsm of the QL causes what Sx?
low back pain, referred to hip and groin, exhalation disfxn of 12th rib, diaphragm restriction
What bad disease must you differentiate QL spasm from?
Renal disease
What are the causes of psoas syndrome?
Irritation of the psoas muscle, nonneutral disfxn of L1 or 2
O/I of psoas?
O-L1-4
I- extends over pubic ramus under inguinal ligament, inserts on lesser trochater on medial side of femur
Psoas innervation?
lumbar nerves 2/3
Psoas action?
flexes trunk on thigh, flexes lumbar spine, and laterall flexes lumbars unilaterally. also shortens and externally rotates leg
What types of things must yourule out before you Dx psoas syndrome?
Since it runs over a lot of important stuff, rule out femoral bursitis, iliac/femoral phlebitis, lymphadenopathy, diverticulitis, cancer of the colon, renal disfxn, or inflmmaton of lower GI
Eitology of psoas syndrome?
sitting in soft chair for slumped in hard chair, bending at waist for long period, then the pt RETURNS TO NORMAL SUDDENLY (still firing when ur sitting)
What type of disfxn will the findings be for L1 and 2 for psoas syndrome?
Type II- they will rotate and sidebend to the same side
If you injure yourself in a nonneutral position, what type of disfxn will you have?
type II
What happens to the pelvis is psoas syndrome?
pelvic side shift to the opposite side
What big nerve can be irritated in psoas syndrome?
Sciatic nerve on opposite side (the opposite piriformis gets irritated from the pelvic shift)
