what hormones made in follicular phase of ovarian cycle?
- FSH stimulates follicle growth
- developing follicles secrete oestrogen (oestradiol)
what hormones cause ovulation?
high oestrogen makes surge of luteinizing hormone which causes release off egg
what hormones are made in luteal phase?
progesterone made by corpus luteum (also makes some oestrogen but main one progesterone)
what hormone is made by fertilised egg when implanted?
hCG = what test for pregnancy
what is hormones produced by placenta?
- human placental lactogen (hPL)
- placental progesterone
- placental oestrogens
what is produced by pituitary in pregnancy?
prolactin (to produce lactogen)
how does gestational diabetes occur?
placental progesterones & hPL →insulin resistance in mother (to increase blood glucose in body to feed the baby)
- if normal & healthy then this no problem
- if predisposed (high BMI etc)→raised blood glucose →gestational diabetes
what are some diabetic complications in pregnancy?
- congenital malformation
- prematurity
- intrauterine growth retardation (baby grows slowly & is small)
- macrosomia (big newborn baby)
- polyhydramnios (too much amniotic fluid)
- intrauterine death
what are complications in neonate with diabetes?
- resp distress = immature lungs
- hypoglycaemia or hypocalcaemia = fits
*also just CNS defects like spina bifida
what trimester does foetus start producing own insulin?
3rd trimester
what is management of diabetes in pregnancy?
- good blood sugar control and good monitoring before & during pregnancy
- need more folic acid = 5mg (usually just need 400 micrograms)
- regular eye checks as accelerated retinopathy
- consider med change like tablets to insulin and ACE inhibitor or statins
- diabetic diet
- aspirin 150mg at 12 weeks since high risk preeclampsia
what drug treatment needed in pregnancy for type 1, type 2 and gestational?
- type 1 needs insulin
- type 2 needs metformin and maybe insulin later
- GDM needs lifestyle, metformin, maybe insulin
what are the 2 most important things for antenatal care in type 1 and type 2 diabetes?
aspirin (150mg) and folic acid (5mg)
what is chance of gestational diabetes progressing to type 2 after pregnancy? how would you prevent?
50% chance to type 2 within 10 years
- keep weight low, healthy diet, exercise, annual fasting glucose (to monitor)
