Diflucan/Fluconzazole

A

IV - do not administer if cloudy of precipitated. Infuse over 1-2 hours - do not exceed 200 mg/hour

Oral - suspension: shake well. May administer without regards to meals

Flagyl/Metronidazole

A

IV - infuse over 30-60 minutes. Avoid contact with aluminum

Oral - take WITH food. Shake suspensino well before adminsitering. Use the calibrated device for dispensing liquids

Topical - apply to clean, dry skin. Wait at least 5 minutes after using lotion

Vaginal - Administer one applicator full. Do not use tampons or douches or engage in vaginal intercourse during the course of treatment. Discard the applicator after each use. If you have a reusable applicator clean with warm soapy water and rinse well.

A

Gentamycin IV - infuse over 30-120 minutes

Tobramycin IV - Intermittent infusion 20-60 minutes. Flush line with saline before and after administration

Keflex/Cephalexin
Kefzol/Cefazolin

A

Cephalexin - take without regard to food. If cause GI distress take with food. If using suspension make sure to shake well before dispensing and use precallibrated syringe to dose medication.

Cefazolin - IV infuse over 3-5 minutes. May allow intermittent infusion over 30-60 minutes. Avoid use with aminoglycosides

Levaquin/Levofloxacin

A

IV - infuse over 60 minutes if under 750 mg. 750 mg infuse over 90 minutes. Rapid infusion can lead to hypotension. Maintain hydration to prevent crystalluria and cylindraria

Oral - tablets: take without regards to meals; suspensions: take 1 hour before or 2 hours after meals. Take 2 hours before or after vitmains, zinx, aluminum, and other supplements

Take with food to improve absorption and decrease effects. Can mix suspension with water, milk, or fruit juice. Shake suspension very well.
