time frame for sydenham’s chorea
starts 4-8 wks after group A strep infxn
usually lasts 2-6 mo, almost always < 2 yrs
chorea minima
comorbidity of ADHD, often seen in Tourette synd
benign (but can interfere w/ fine motor skills)
NMDA encephalitis
IgG to NMDA receptor
MC autoimmune encephalitis
AMS > seizures > dyskinesia (facial stereotypies MC)
tx w/ intensive prolonged steroids, IVIG, immune modulation
characteristics of dystonia
involuntary / insuppressable
stereotyped, twisting
exacerbated by volitional mvmt or emotion
hypertonia during mvmt (baseline tone can be nl)
reflexes nl to inc
dyskinetic CP
varied mvmts - dystonia, chorea, athetosis, ballism
avg IQ** often unrecognized though
major causes - kernicterus, near term hypoxia, basal ganglia CVA
Opsoclonus myoclonus
assoc w/ neuroblastoma
twitching of face/eyes and feet
hyperekplexia
excessive startle response
non extinguishable glabella reflex
often causes tonic posturing
