Malabsorption
What is the actual offending agent in celiac disease?
Gliadin
A patient presents with diarrhea and bloating with weight loss. Stool sample show steatorrhea and exam shows significant malabsorption of multiple nutrients.
Celiac Disease
What is gold standard for celiac diagnosis?
Endoscopic biopsy of the small intestine
A serological study for screening celiac disease (but not diagnosis).

Total elimination of gluten
A common BOARD associated manifestation w/Celiac disease.
Dermatitis Herpetiformis
If a patient with celiac has being doing well on a gluten-free diet and suddenly becomes a non-responder, what could be going on?
Intestinal Lymphoma
Testing for tropical sprue
O & P testing 3x (-)
Tropical Sprue Tx
Gluten-free diets DO NOT improve
Tetracycline x 6 mo + folic acid supplementation
People who typically have short bowel syndrome.
1) 1/2 small intestine removed
2) sig. damage of small intestine
3) poor motility
Med for short bowel syndrome
Gattex (Teduglutide)
W/ H2 blockers/PPIs, hypomotility agents, supplements
Antibiotics tx for bacterial overgrowth
Metronidazole (Flagyl)-ETOH!
Cipro
*May require ongoing intermittent tx.
