11B: Peds Pulmonary Flashcards
When is surfactant developed?
24 weeks gestation - fully developed at 35 weeks
Prior to how many weeks is V/Q mismatch a serious threat?
26 weeks - capillary structure around lungs not developed enough
What are pulmonary complications with premature birth?
- Underdeveloped diaphragm
- Decreased/absent cough reflex
- Lack of body fat leads to increased O2 consumption to maintain body temp
What is Tracheal Atresia?
Trachea does not develop correctly and ends into nothing or has intersection with esophagus
What are symptoms of Tracheal Atresia?
Respiratory distress leading to CPR and immediate surgery
What are interventions for Tracheal Atresia?
Surgery to attempt to connect trachea into lung system
What is Bronchopulmonary Atresia?
Poor formation of bronchus going to one or more bronchopulmonary segments
What does Bronchopulmonary Atresia lead to?
Increased fluid and hyperinflation
What are symptoms of Bronchopulmonary Atresia?
- V/Q mismatch
- Poor eating
- Fatigue
- Increased RR
- Fluid sounds with auscultation
What are interventions for Bronchopulmonary Atresia?
Surgery to resection and rebuild that bronchus
What are symptoms of chest or abdominal wall anomalies?
- SOB
- Fatigue
- FTT
What is the most common pulmonary disorder in preemies?
Intant RDS - respiratory distress syndrome
When does Infant RDS form?
Due to incomplete lung formation, not enough alveoli developed yet, and not enough surfactant
What are symptoms of Infant RDS?
- Poor ventilation
- Increased RR
- Cyanosis
What are interventions for Infant RDS?
Ventilation and meds to increase surfactant and decrease inflammation
