11. Penis neoplasms Flashcards
1
Q
Types of penis neoplasms
A
- Condyloma acuminatum
2. Squamous cell carcinoma of the penis
2
Q
Definition of condyloma acuminatum
A
Benign sexually-transmitted tumour of the penis
3
Q
Associations with condyloma acuminatum
A
HPV types 6 & 11
4
Q
Morphology of condyloma acuminatum
A
- Grossly:
- Lesion found on the coronal sulcus, on the inner surface of the prepuce
- Single/multiple, sessile/pedunculated red papillary excrescences - Histologically:
- Branching, villous papillary stroma
- Covered by epithelium which may have superficial hyperkeratosis & thickening of the underlying epidermis (acanthosis)
- Normal orderly maturation of epithelial cells is present
- Cytoplasmic vacuolization of squamous cells (koilocytosis) characteristic of HPV infection
5
Q
Epidemiology & associations with squamous cell carcinoma of the penis
A
- Geographical variations (10-20% of male malignancies in high risk areas)
- Circumcision is protective (rare in Jews & Muslims)
- HPV types 16 & 18
6
Q
Morphology of squamous cell carcinoma of the penis
A
- Grossly:
- Lesion on the glans of the inner surface of prepuce
near the coronal sulcus - Histologically: (2 patterns)
- Papillary (simulates condyloma acuminatum, may produce cauliflower-like mass)
- Flat (areas of epithelial thickening with graying & fissuring of mucosal surface)
7
Q
Clinical features of squamous cell carcinoma of the penis
A
- Slow growing, locally invasive
- Metastases to inguinal & iliac lymph nodes indicates a poor prognosis
- Pain develops after tumours undergo secondary ulceration & infection
