at the 2nd intercostal space superior to the third rib
followed by chest tube placement
Where would one insert a chest tube?
at the anterior axillary line or mid axillary line of the 4th intercostal space superior to the fifth rib
where would you see the lower margin of an endotracheal tube on a chest x-ray?
T4
What’s the landmark for a thoracentesis?
7th intercostal space
What would pursed lips be indicative of in a patient?
COPD, labored breathing
What might a deviated trachea indicate?
pneumothorax
Where should hands be placed to chest thoracic expansion?
thumbs should be placed at the level of the 10th rib
then ask pt to inhale deeply and observe thumbs
What might cause a decreased or absent fremitus?
COPD
Pleural effusions
pneumothorax
Fibrosis
What might cause increased fremitus?
pneumonia (increased transmission)
What are the five types of percussion notes you may observe?
Flat Dull Resonant Hyperresonant Tympanic (abdominal)
What might unilateral hyperresonance suggest?
large pneumothorax
or
large air-filled bulla in lung
How should the patient be instructed to breath when auscultating?
deeps breaths with an open mouth
