101 Flashcards
101.1
What are the 5 steps of ORM?
Identify Hazards Assessing Hazards Making Risk Decisions Implementing Controls Supervising
101.1A
Explain Identifying Hazards
A hazard is any condition with the potential to negatively impact mission accomplishment or cause injury, death, or property damage.
- Analyze the mission
- List the Hazards
- Determine the Hazard Root Cause
101.1B
Explain Assessing Hazards
- Severity
- Probability
- Complete risk assessment
- Risk assessment pitfalls
101.1C
Explain making risk decisions
- Identify control options
- Determine control effects
- Make risk decisions
101.1D
Explain implementing controls
Requires that the plan is clearly communicated to all the involved personnel, accountability is established, and necessary support is provided.
101.1E
Explain Supervising
Monitor effectiveness of risk controls, determine need for further assessment of mission or task due to an unanticipated change, capture lessons learned positive and negative.
101.3
Define high risk course
A known or unknown condition or state where an elevated probability of loss or an increased level of severity is likely or imminent.
101.4
Discuss types of course considered high risk
Expose screw, staff, students or assets to the potential risks of death, permanent disability, or loss during training
101.5A
Voluntary training
Training in which a sailor has voluntary enrolled and has the ability to Drop On Request and return to original rating
101.5B
Involuntary course
Training in which a sailor is enrolled thru accession training or follow on specialty skills school
101.6
What is DOR and what are the procedures
High risk training course desires to quit or DOR, the student need only make intentions know. Student will be immediately removed from training area, written summary entered in students service record and copy maintained in commands permanent records.
101.7
What is TTO and procedures
Any situation when student or instructors express concern for safety or a need to clarify procedures or requirements. Training will immediately stop until situation or condition is returned to a safe state.
101.8
Discuss purpose of EAP and how often it must be reviewed and practiced
Internal plan implemented immediately upon advent of a mishap to aid involved persons and to control the scene.
Simple checklist of responses and immediate action.
Quarterly walkthrough.
Annual full exercise.
101.9
Periodic requirements for safety stand downs.
Annually and at COs discretion
101.10A
Discuss mishap reporting
Report and record all training related mishaps and injuries per OPNAVINST 5102.1D
Training mishap is any mishap that occurs to instructors, students, or assets while conducting training as result of curriculum or environment
101.10B
Discuss Site Augment plans
Ensure instructors are certified to instruct utilizing safety requirements, precautions, and safeguards relative to the course they teach. Includes completion of CUIT and site augment plan if required.
101.10C
Special course indicator code
Voluntary training. High risk training that is designated voluntary per resource sponsor and CCA.
Code A or D in CeTARS.
101.10D
Core unique instructor training
(HIGH RISK ONLY) CUIT designed to prepare instructor to teach in high risk course. Items must apply universally to all sites where Course is taught.
101.10E
Instructor screening process
1.Service record screening(adverse administrative entries, below average evals, NJP, shall be told to CO before interview)
2. Physical requirements
3. Medical officer interview, record review and questionnaire
4. COs interview
OPNAVINST 1500.75 Policy and procedures for high risk training
101.10F
Evaluation process
Establish an evaluation program that assesses high-risk instructors in classroom and laboratory
or field settings on a recurrent basis, in percentages commensurate with the amount of time
spent instructing in those environments.
101.10G
Training Safety Officer
HM1 Robichaux
101.11
Discuss the importance of heat/cold stress, monitoring, and control in a training
The importance is based upon prior conditioning, activity level of the training to ensure Sailors
environment
are prepared to participate in the activity and mitigate potential loss of life
