Right Client
2 identifiers (compare against EHR)
Right Medication
Name & form are correct & appropriate-check expire date
Check med label 3 times against MAR.
Always know rationale behind the prescription
Right Dose
Ordered & safe (calculation)
Consider patient’s renal & liver function as well as indication/rationale for use
Right Route
Ordered & appropriate
Each route has different safety issues to consider
Right Time
MAR, last time given, frequency is appropriate
Evaluate specific of medication but also what times work for the patient
Right Assessment
Allergies, contraindications, appropriate for patient, interactions, labs, VS
Right Documentation
Pertinent assessment findings, time given
Document medication as given only after administered
Right to refuse
Find out why, provide education, document & inform provider
Right to education
Name, purpose, potential adverse effects
Right evaluation
